If your school Athletic Trainer has provided you with a testing code please enter it in the box below. If you wish to take the test and do not have a code, there is a $5.00 fee for the test. You may pay by using the paypal buy button. Upon successful code validation or completion of payment you will automatically be redirected to the Baseline Test website. Note that if you elect to purchase the test via PayPal, you will need a testing code to proceed to the test.
