Prior to completing the ONLINE PRE-REGISTRATION, contact Methodist Sports Medicine/The Orthopedic Specialists at 317.817.1200 OR complete and submit the APPOINTMENT REQUEST.
You must first have an actual appointment to complete the ONLINE PRE-REGISTRATION form. Upon receipt of your appointment request via phone or the online APPOINTMENT REQUEST you will be contacted with information about your specific appointment date, time and location.
