Insurance Information
Remember to bring your insurance cards, and we will be happy to file your private health insurance for you. Keep in mind that you are responsible at the time of service for any co-payment or deductible.
If yours is a worker’s compensation case, we will need written authorization from your employer to treat you, and we will also need complete billing information.
If yours is a liability case, such as an auto accident, we will file your claim with your private health insurance, or you may choose to be responsible for payment at the time of service and submit all charges to the appropriate liability carrier.
